The 43-year-old American, who was returning for her 24th singles appearance at The Championships, is facing difficult opposition in Ukraine’s Elina Svitolina.
But things turned ugly with the score just 2-1 in the first set as Williams came into the net to play a volley, slipping on the grass and extending her already-strapped up knee.
There were audible gasps from around the crowd on Centre Court as five-time Wimbledon champion Williams screamed in pain, before being immediately helped by the umpire and medical staff.
After a few minutes, she was able to stand under her own power and called for a trainer to attend to her at courtside.

Following a short break in play, Williams attempted to continue the match, but was still in visible pain as she grimaced through the next few games.
She went on to lose three straight games, as fans speculated whether she was going to be able to continue, before she rallied to bring it back to 4-3, a break down.
Williams has won five Wimbledon singles titles in her career, appearing 24 times in the competition, but she missed 2022 and many believed her time in SW19 had come to an end.
She was given a wildcard entry after beating world No 48 Camila Giorgi in Birmingham at a Wimbledon warm-up tournament, and went on to suggest she could play until she is 50 in interviews leading up to the tournament.
